● E-citizen pre-filled application forms
● Copy of old passport (if available)
● One coloured photograph
● Sworn affidavit from a lawyer
● Police abstract
● An explanation letter to explain the circumstances of the loss

How to Apply
- All passport applications should be made via www.ecitizen.go.ke
- Print out the form, date the form, add any documents and photographs that are needed
- Submit your application form in person at the Immigration offices (Nyayo House Nairobi, Mombasa, Kisumu, Embu, Nakuru, Eldoret, Kisii) for biometric registration
NOTE
● Manual Applications will not be accepted.
● New Applications can only be lodged in Kenya
